Your First Visit

To prepare for your visit with Dr Schluter, please remember to bring with you:

1. Referral letter from your GP or other Healthcare Professional
2. The name of the clinic where you had your xrays taken.
3. Xrays or scans you have been given on film or CD disc
4. A full list of your current medications and any known allergies
5. Any Medical Specialist letters or reports for other disorders
6. Operation reports for related surgeries
7. ACC claim number (if applicable )
8. Medical Insurance details (if applicable )
